Community & Wellbeing Support Worker Apprenticeship Level 3

This Standard is aimed at staff who are committed to developing their skills and knowledge. It allows existing employees who are keen to move into a Community Health role to upskill, as well as supporting the development of staff already in a similar role. We ensure your employees receive the necessary knowledge, skills and behaviours (KSBs) that meets the required industry standards.  The Standard also provides them with the skillset to support the ongoing development of your team, creating more flexibility in the general practice workforce. 

Grades 9-4 / A-C in maths and English GCSE or equivalent. Learners without the required grades, will be required to complete a maths and English assessment, before being accepted onto the programme.  

Community Health and Wellbeing Workers are found in different organisations, including local government, the NHS, and other funders such as voluntary, community and social enterprise organisations. Job titles include Care or Service Navigator, Community Connector, Community Health Champion, Health Trainer, Live Well Coach, Social Prescribing Link Worker. 

They are a rapidly expanding workforce, supporting the increasing emphasis across government departments on improving the health of local people and communities. 

The job purpose is to work in partnership with individuals and their communities to identify and address health and wellbeing needs, improve health, prevent ill-health and reduce inequalities. They:  

  • address the causes of poor health and wellbeing by taking an holistic approach regarding physical, mental, emotional and social health and wellbeing and resilience 

  • work with individuals, groups and communities to identify what matters to them 

  • understand the local and accessible services and resources available, and signpost individuals to support their health and wellbeing needs 

  • identify gaps in available services and resources preventing individuals and communities from achieving optimal health and wellbeing 

  • build relationships with local organisations and groups 

Throughout the programme, the college will provide expert tuition for the learners from highly trained staff. A qualified trainer/assessor will provide an induction, group teaching sessions, coaching, regular assessments and ongoing progress reviews to both the employee and their line manager.
